94 Stories Informed Consent Agreement

“94 Stories” (Re-Connecting the Twin Cities) is an oral history project, led by Our Streets MN, to expand its transportation history and advocacy work to St. Paul communities affected by legacy transportation decisions. Oral histories are extensive and recorded interviews in which a participant shares their stories and memories for current and future generations to hear. This project will also collect group conversations and shorter interviews at community events.

I voluntarily agree to participate in the 94 Stories Oral History Project (“the Project”) and hereby donate and convey my oral history interview, digitized images, and/or other materials to the Project and to Our Streets. Our Streets public historian and project staff will then transfer and donate all interview materials (recordings. transcripts. memorabilia. etc.) to the Minnesota Historical Society as the final repository, with copies of interview transcripts also being housed with the Minnesota Digital Library.

I understand that I will have the opportunity to review my interview before it is placed in the repository and made available to the public. I further understand that the following items may be created from my interview (if applicable):

  • an audio and/or video recording
  • an edited transcript and summary
  • a photograph of me
  • copies of any additional documents or photographs that I wish to share.

I understand that my interview (and other items I provide to the Project staff) may be distributed to the public for educational purposes, including formats such as print, public programming, documentary and short films, and the Internet.

I agree to freely share my interview and other donated images and items under the terms of a Creative Commons Attribution-NonCommercial-ShareAlike 3.0 Unported License. This means that I retain the copyright but that the public may freely copy, modify, and share these items for non-commercial purposes under the same terms if they include the original source information.

In return. the interviewer agrees to send one free copy of the interview recording, transcript, and related items to the address I share in the form.
